Otterbein is seeking a <strong>Hospice RN Case Manager,</strong> to assist in the growth of our Hospice ministry. Otterbein Hospice compliments already strong independent senior services, outpatient capabilities and clinical facilities at Otterbein.
Hospice is a philosophy of care for those facing life-limiting illnesses, as well as their families and caregivers. The focus of hospice care is to help patients experience peace, comfort, and quality of life while also offering support and services to their family and caregivers.

<strong>Shift: Full-Time 8a-5p, Monday - Friday Plus on call rotation as scheduled</strong>
Responsibilities
Otterbein is seeking an enthusiastic and professional <strong>Hospice RN Case Manager</strong>
- This role is responsible for primary caseload.
- This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.
Qualifications
- Education: Bachelor’s degree in nursing preferred. Graduate of approved nursing program
- Licensure/Certification: Valid Ohio Registered Nurse (RN) license
- Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice and Case Management experience preferred
- Experience: Experience with end-of-life diagnosis preferred
