About the Role
Integrate behavioral health care into primary care settings to make mental health support more accessible, coordinated, and effective for patients. As a Behavioral Health Care Manager, you'll support individuals experiencing common mental health and substance use concerns by providing brief, evidence-based interventions while working closely with medical providers and psychiatric consultants as part of a collaborative care team.
Responsibilities
- Manage a defined patient caseload, conducting screenings, providing short-term interventions, and tracking treatment progress over time to ensure patients are improving and staying engaged in care.
- Work in close coordination with physicians, psychiatrists, and other behavioral health providers to guide treatment decisions, support care transitions when needed, and connect patients with additional resources.
Location
- Office Location: Jackson, MI
- Main Service Area: Jackson and surrounding areas
Hours
- Monday to Friday, 8 a.m. to 5 p.m.
Required Qualification
- Formal education or specialized training in behavioral health (social work, nursing, or psychology)
- Minimum 2 years of social work or behavioral health experience
- Experience screening for common mental health and substance use disorders
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Essential Skills and Competencies
- Working knowledge of differential diagnosis for common mental health and substance use disorders
- Experience with assessment and treatment planning for mental health and substance use concerns
- Proficiency in evidence-based psychosocial treatments and brief behavioral interventions
- Strong therapeutic engagement skills with ability to build rapport quickly
- Experience with evidence-based counseling techniques
- Basic knowledge of psychopharmacology for common mental health disorders (within scope of practice)
- Ability to work effectively in collaborative care team model
