About the role
Department & Position Overview:
FreedomCare's Georgia operations support Structured Family Caregiving (SFC), a Medicaid home and community-based service option that helps eligible individuals receive daily care from a qualified live-in family caregiver, with coaching and oversight to support safe care at home.
The Care Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members and Case Management Agency, and provides education to caregivers and patients.
This is a full-time, field-based position that requires frequent travel to patients' homes in Jonesboro, McDonough, Lawrenceville, and Morrow counties.
Every Day You Will:
- Conduct initial and semi-annual in-person visits, in-person RN supervisory visits, and coordinate timely service activation, ensuring all required documentation, approvals, and standard procedures are completed.
- Complete self-scheduling for in-person visits according to company policy.
- Complete a minimum of 16-20 in-person visits weekly. Fluctuation may occur dependent on established needs and territory.
- Complete in-person visit documentation and care coordination the same business day as the visit.
- Follow field safety protocols, including PPE use when indicated, equipment and privacy rules, and HIPAA requirements.
- Drive independently to and from patient residences. Drive time can be up to 2 hours, with occasional need outside the assigned territory.
- Develop, implement, and manage individualized coaching care plans, documenting revisions as patient needs or conditions change.
- Educate patients, caregivers, and internal teams regarding care needs, safety considerations, service requirements, caregiver responsibilities, and next steps.
- Coordinate patient care through timely communication and documentation with clinical disciplines, internal partners, case management agencies, and community resources.
- Provide ongoing education and coaching to support best practices, promote caregiver competency, and connect patients and families with resources.
- Provide SFC-specific clinical consultation and guidance to Health Coaches, Care Specialists, and caregivers when concerns are identified.
- Review caregiver daily notes and related documentation for each assigned active SFC case at least every 14 calendar days, documenting each review in an auditable manner.
- Identify and timely escalate changes in condition, health or safety concerns, unmet care needs, service gaps, incidents, and urgent concerns per clinical protocols, and complete follow-up actions within required timeframes.
- Complete requirements using approved documentation methods and systems, phone calls, and customer outreach tools (Salesforce, TalkDesk, Email, Teams, CareConnect, MedFlyt).
- Identify clinical process gaps, documentation risks, and recurring concerns, and support policy implementation, compliance reviews, audit preparation, record validation, and corrective follow-up.
- Maintain compliance with federal and Georgia requirements, Medicaid and EDWP program requirements, CCSP and SOURCE requirements, SFC service requirements, professional nursing standards, and company policies.
- Attend team meetings virtually or in-person as requested.
- Escalate any unsafe or concerning customer or staff issues immediately per company policy.
- Perform other related duties as assigned by the supervisor.
Credentials/Certifications:
- Diploma from an accredited nursing program and a valid, active Registered Nurse (RN) license in the State of Georgia (Georgia compact RN licenses are allowed).
- A minimum of two years of recent nursing experience supporting adults or geriatrics and/or adults with disabilities preferred.
- Experience supporting Medicaid waiver programs, home and community-based services, clinical documentation review, care coordination, compliance monitoring, or clinical escalation workflows preferred.
- Valid driver's license and car insurance required, with reliable transportation.
- Valid CPR/first aid certification must be active or obtained prior to starting the position.
Qualifications:

- Ability to follow established scripts, workflows, policies, procedures, quality standards, and regulatory requirements.
- Strong active listening skills, with the ability to understand patient needs, clarify concerns, and escalate when appropriate.
- Effective verbal and written communication skills, with the ability to summarize interactions, document case activity, and communicate case status and next steps.
- Strong typing and computer proficiency, including contact center technology, CRM systems, telephone platforms, and other software.
- Effective problem-solving and critical thinking, with sound judgment and the ability to escalate when necessary.
- Dependable and punctual, with a commitment to consistent attendance and schedule adherence.
- Ability to adapt to changing priorities, processes, systems, and business needs in a fast-paced environment.
- Receptive to coaching and feedback, with a commitment to continuous improvement.
- Ability to collaborate as part of a team while working independently within established guidelines.
- Strong clinical judgment and critical thinking skills.
- Proficiency with electronic documentation systems, CRM platforms, Microsoft Office applications, and other systems used for clinical-oversight activities.

Compensation Range:
The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment. Compensation decisions depend on factors which may include skill set, years of relevant experience, education, location, and licensure/certifications.
