About the role
Department & Position Overview:
FreedomCare's Georgia operations support Structured Family Caregiving (SFC), a Medicaid home and community-based service option that helps eligible individuals receive daily care from a qualified live-in family caregiver, with coaching and oversight to support safe care at home.
The Care Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. This full-time field-based position requires frequent travel to patients' homes in Jonesboro, Clayton, Lawrenceville and Morrow area counties.
Every Day You Will:
- Conduct initial and semi-annual in-person visits, in-person RN supervisory visits, and coordinate timely service activation, ensuring documentation, approvals, and standard procedures are completed.
- Complete a minimum of 16-20 in-person visits weekly, with fluctuation dependent on established needs and territory.
- Complete in-person visit documentation and care coordination the same business day.
- Follow field safety protocols, PPE requirements, equipment and privacy rules, and HIPAA laws.
- Drive self independently to patient residences; drive time can be up to 2 hours.
- Develop, implement, and manage individualized coaching care plans, documenting revisions as needs change.
- Educate patients, caregivers, and internal teams on care needs, safety considerations, service requirements, and next steps.
- Coordinate patient care with clinical disciplines, internal partners, case management agencies, and community resources.
- Provide SFC-specific clinical consultation and guidance to Health Coaches, Care Specialists, and caregivers.
- Review caregiver daily notes for each assigned active SFC case at least every 14 calendar days and respond to identified concerns.
- Identify and escalate changes in condition, health or safety concerns, unmet care needs, and service gaps in accordance with clinical protocols.
- Use only approved documentation and outreach systems (Salesforce, TalkDesk, Email, Teams, CareConnect, MedFlyt).
- Identify clinical process gaps and compliance risks, and support policy implementation, compliance reviews, and audit preparation.
- Maintain compliance with federal, Georgia, Medicaid, EDWP, CCSP, SOURCE, SFC, and professional nursing requirements.
- Attend team meetings virtually or in person as requested.
- Escalate unsafe or concerning customer or staff issues immediately per company policy.
- Perform other related duties as assigned by the supervisor.
Credentials/Certifications:
- Diploma from an accredited nursing program and a valid, active Registered Nurse (RN) license in the State of Georgia (Georgia compact RN licenses are allowed).
- Preferred: a minimum of two years of recent nursing experience supporting adults, geriatrics, and/or adults with disabilities.
- Preferred: experience with Medicaid waiver programs, home and community-based services, clinical documentation review, care coordination, compliance monitoring, or clinical escalation workflows.
- Valid driver's license and car insurance required, with reliable transportation.
- Valid CPR/first aid certification must be active or obtained prior to starting the position.
Qualifications:
- Demonstrated commitment to high-level customer service, with empathy, patience, professionalism, and respect.
- Ability to follow established scripts, workflows, policies, procedures, quality standards, and regulatory requirements.
- Strong active listening, verbal and written communication, problem-solving, and critical thinking skills.
- Strong clinical judgment.
- Strong typing and computer proficiency, including contact center technology, CRM systems, and telephone platforms.
- Dependable and punctual, with ability to adapt to changing priorities in a fast-paced environment.
- Receptive to coaching and feedback, and able to collaborate as part of a team while working independently.
- Proficiency with electronic documentation systems, CRM platforms, and Microsoft Office applications.

Compensation Range:
The base pay range for this role is between $80,000 and $90,000 per year at the commencement of the first year of employment. Compensation depends on factors such as skill set, years of relevant experience, education, location, and licensure/certifications.
